Use this worksheet to review the documents and terms of policies you already own. Complete one copy for each policy and confirm questions or requirements directly with your insurer.
Policy number (optional): ____________________ Review date: ____________________
Locate the current policy schedule and wording. Keep the latest schedule, endorsements, renewal notices, and premium receipts together.
Confirm the insured people, property, or vehicle. Check names, dates of birth, addresses, vehicle details, and other identifying information against your records.
Review the coverage amount and benefits. Note limits, sum insured, insured declared value where relevant, and any benefit sub-limits stated in the policy.
Read exclusions, waiting periods, and conditions. Record the clauses that may apply to your likely needs and ask the insurer about wording you do not understand.
Check disclosures in the proposal and application. Keep a copy of the submitted proposal and tell the insurer if you find a material error; do not alter records yourself.
Verify nominee or beneficiary details where applicable. Confirm the recorded details and ask the insurer how to update them after a life event.
List riders, add-ons, co-payments, and deductibles. Note their costs, limits, and conditions separately from the base policy.
Check renewal date, premium, and payment method. Save reminders and verify any lapse, grace-period, or renewal consequences in the policy wording.
For health cover, confirm care and claim arrangements. Check the current network information, pre-authorization instructions, reimbursement process, and document requirements with the insurer.
For motor cover, review vehicle value and selected covers. Check the insured declared value, third-party and own-damage cover, deductibles, and selected add-ons against your policy.
For life cover, keep claim and contact information accessible. Tell the people who may need it where the policy and premium records are stored and how to contact the insurer.
Keep a claim-ready record. Save the insurer's claim steps, customer-care and grievance contacts, policy documents, and receipts in a place your family can find.
Questions to send your insurer
1. Which policy clause explains this limit or exclusion?
2. Which documents are required for my situation?
3. How can I request a change and receive written confirmation?
